Employment letter or Contract (if working on contract basis)
Approval letter from Company for the name of Company to be printed on Applicant’s Business Card

Copy of Business registration
Copy of IC of sole proprietor/partners
Copy of latest 12 months Bank statement/financial statements

Limited or Private Limited Companies
